Depression screening questionnaire is a clinical practice performed by depression consultants where a form is provided to depressed patients in order to analyse there behaviors to natural activities.
Depression screening questionnaire form should have detailed questions to get exact answers, the PHQ-9 and (PHQ)-2 are the standard health screening forms provided in US. The fact is that a patient can also provide wrong answers, its good practice to also have live interaction with most of the patients and ask few oral questions and make a note. As kids not sure which is answer is correct. The doctors today should not just relay on questionnaires, they need to study the patient behavior, attitude,
skills, manners, respect etc..
For an example: a friend of mine who was really good had a depression, he was fine when talking to single but he was depressed to go in groups. He was not comfortable when talking in groups, stage fear. He even was not willing to attend the interviews. He had a fear when he see huge crowd. When i ask him to come for an interview he gets depressed and makes excuses not to attend the interview. He always loves to be single. Right now he works as a freelancer from home and earns a little.
When his dad took him to a physicist he asked plenty of questions which took over an hour. The doctors said that he loves to interact with many, but when he sees people in groups he is feared that something will happen. He also said that during his childhood he has lost his mother and the father has restricted him to stay alone at home. This has made him love loneliness.
So depression is always caused with incidents happens in childhood, parents should be behind kids and ensure to talk positive about incidents happening in world. I'm sorry but this is all wrong! The conclusions are wrong and whoever wrote this does not have a true understanding of what depression is, what causes it, and how to help someone.
